冷凍保存血管移植物已廣泛應用於肝膽胰外科進階手術中,特別適用於需進行血管重建的情況,如肝門膽管癌、局部晚期胰臟癌,以及複雜的肝臟移植手術。在需合併門靜脈、肝動脈或下腔靜脈切除的情形下,冷凍保存的動脈與靜脈異體血管提供良好的結構穩定性、免疫相容性與即時取得性。於活體肝臟移植或再次移植中,當遇到血管解剖異常或術後沾黏難以使用自體血管時,冷凍血管可作為重要替代選擇。本演講將分享臨床適應症、手術技巧、血管選擇與術後成效,結合文獻與本中心經驗,強調血管通暢率、感染控制、免疫學考量及腫瘤長期安全性。經適當病例選擇與熟練操作,冷凍保存血管移植物可提升肝膽胰腫瘤根除手術及肝臟移植中大血管重建的安全性與可行性。
Cryopreserved vascular grafts have become vital in advanced hepatobiliary and pancreatic (HBP) surgery, especially when vascular reconstruction is required. In cases such as hilar cholangiocarcinoma, locally advanced pancreatic cancer, or complex liver transplantation, en bloc vascular resection with reconstruction may be necessary. Cryopreserved arterial and venous allografts provide structural integrity, immunologic tolerance, and immediate availability, making them suitable for portal vein, hepatic artery, and inferior vena cava replacement. In liver transplantation, these grafts are especially useful in living donor liver transplantation or re-transplantation, where anatomical variation or reoperative adhesions preclude native vessel use. This presentation will review clinical indications, surgical techniques, graft selection, and postoperative outcomes based on institutional experience and literature. Emphasis will be placed on graft patency, infection control, immunologic compatibility, and oncological safety. With appropriate patient selection, cryopreserved vascular grafts enhance the feasibility and safety of complex HBP resections and liver transplantation involving major vascular reconstruction.
